Everything Case Managers need to make a referral, in one place.
ILO exists to work well with case managers – clear answers, fast turnaround, and a team that tells you honestly whether or not we’re the right fit.
HOW WE WORK TOGETHER
A high bar for responsiveness.
We hold ourselves to a high standard for response time – on every kind of communication, not just referrals. If we can’t take on a referral, we always tell you why. When a service isn’t the right fit, we’ll help point you toward the right resource, even outside of ILO.

What to Have Ready
Just three things to start: the individuals name, the city they live in, and their current living situation. We’ll help you figure out the rest – documentation like a support plan comes later, if needed.
What Happens After a Referral
Danielle or Nicole respond by email, then the division director joins to finalize a decision. If ILO can’t accept the referral, we’ll always tell you why – you’re never left without a response.

Your job asks a lot of you, so we try not to. That’s why we move fast, stay honest, and treat every referral like it matters – because it does.
FAQ
-
An authorized waiver case manager — that's who referrals are built for, though we know there are some nuanced situations. If you're a family member or another kind of referring party rather than a waiver case manager, please use our general contact form instead and we'll point you in the right direction.
-
Yes — that's genuinely encouraged. A lot of referrals really do start as just a conversation, and we're happy to talk through fit before anything is formal.
-
We'll always tell you directly, along with the reasoning — no one is left without a response. Where it makes sense, we'll also suggest a related service that could work in the meantime, or point you toward another resource entirely.
-
No — that's not something we publish online. It doesn't come up often, and when it's needed, it comes through naturally as part of the referral process once we're in touch directly.
